Employer Intake Packet is for the parent or guardian of the person receiving care.
Employee Intake Packet is for all individuals who are earning wages while providing care (the employer is also required to fill this out if they are earning wages).
W-4 is a tax form that employees use to provide information on their withholding allowances for federal income tax.
The Direct Deposit Authorization Form provides Direct deposit information so money can be deposited quickly and securely.
